Patient Forms

For your convenience, you can download our consultation form and complete it prior to your first visit. Then, bring the form with you when you come in for your appointment. If you choose to fill out the consultation form at our office, please arrive 10-15 minutes before your appointment to allow enough time to get them completed.

Our forms are PDF files. To download and print the forms, you’ll need the free Adobe Acrobat Reader program.

Physio & Therapy is approved by all major UK insurance companies.

For your convenience our administration team are able to send your invoices directly to some of the insurance companies. All we require is your policy number, authorisation code and how much excess you need to pay, if any. Please check with reception when making your appointment.

All our physiotherapists are Chartered and Heatlh Professioanls Council (HPC) registered.

Patients are advised to keep all receipts for physiotherapy in order to claim monies from their insurance company.

If your insurance company is not on the list above, dont worry, the list is not exhaustive and we are likely to have agreements in place to treat you. Always call to check: 01434 307989


Cancellation Policy

Repeated late cancellations and no-shows are disruptive to the optimal delivery of care and service to you and our other patients. Please notify us at least 24 hours prior to your appointment if you are unable to attend or if you wish to move your appointment time.

Failure to comply with our cancellation policy will result in 50% of the appointment price being charged.

If you are claiming from a private medical insurer, they may not pay this fee and you will therefore be liable for this payment.

Exceptions will be made at staff discretion, for exceptional circumstances or genuine illness.
